Detroit Recovery Project, Inc. (DRP) is a Certified Community Behavioral Health Clinic (CCBHC) that provides integrated behavioral health and recovery support services for individuals experiencing mental health and substance use challenges. Our recovery ecosystem focuses on five key areas: Spirituality Employment Education & Training Family & Recovery Support About the Opportunity: Detroit Recovery Project is seeking a Youth Case Manager to support young adults ages 18–24 who are engaged in substance use disorder (SUD) and co-occurring mental health recovery services. This position plays a vital role in helping participants access healthcare, behavioral health services, housing resources, educational opportunities, workforce development programs, and other community supports that promote long-term recovery and independence. The Youth Case Manager will work closely with participants, clinicians, peer recovery coaches, residential staff, community partners, and the Detroit Wayne Integrated Health Network (DWIHN) to ensure coordinated, person-centered care and successful linkage to services. Compensation The pay range for this position is $21.63-$24.04 per hour, based on non-discriminatory factors such as education, experience, certifications, technical skills, and overall qualifications. This is a full-time, non-exempt position and is eligible for overtime compensation in accordance with applicable laws and organizational policies. What We're Looking For: Detroit Recovery Project is seeking an empathetic, organized, and mission-driven professional with experience providing case management and supportive services to youth and young adults. The ideal candidate understands recovery-oriented systems of care and is passionate about helping transition-age youth overcome barriers and achieve lasting success. Candidates with experience working in behavioral health, substance use treatment, youth services, housing programs, community outreach, or recovery support services are strongly encouraged to apply. What You'll Do: Conduct intake assessments, psychosocial evaluations, and ongoing needs assessments to identify participant strengths, goals, and service needs. Develop individualized service plans rooted in person-centered, trauma-informed, and culturally responsive care. Assist participants in identifying barriers to success and implementing measurable goals and objectives that promote recovery, stability, and independence. Provide comprehensive case management services, including assessment, service planning, discharge planning, and direct coordination with the Detroit Wayne Integrated Health Network (DWIHN) to ensure continuity of care and successful linkage to community-based behavioral health and support services. Coordinate referrals and connect participants to medical care, behavioral health services, housing resources, educational opportunities, workforce development programs, and other community supports. Provide ongoing advocacy, monitoring, and follow-up to ensure continuity of care and progress toward individualized goals. Monitor participant engagement, attendance, and progress while maintaining accurate and timely documentation. Collaborate with clinicians, peer recovery coaches, residential staff, and community partners to ensure integrated and coordinated service delivery. Facilitate life skills development, recovery-focused coaching, and goal-setting activities as appropriate. Assist with crisis intervention and de-escalation efforts in accordance with agency policies and procedures. Coordinate services with community organizations, referral partners, and external providers. Administer required pre- and post-program evaluation tools and outcome measures. Maintain accurate participant records and enter data into organizational and funder-required systems in a timely manner. Participate in staff meetings, trainings, case consultations, and quality improvement initiatives. Demonstrate and actively promote an understanding of and commitment to the mission, vision, and values of Detroit Recovery Project. Perform other duties as assigned. Qualifications: Bachelor's degree in Social Work, Psychology, Human Services, Public Health, or a related field preferred; equivalent experience may be considered. Minimum of one (1) year of professional experience providing case management or related supportive services required. Experience working with youth or transitional-aged young adults, particularly those experiencing substance use disorders and co-occurring mental health conditions, strongly preferred. Knowledge of community resources, recovery-oriented systems of care, and person-centered planning practices. Training in Motivational Interviewing preferred. Strong communication, interpersonal, crisis intervention, and problem-solving skills. Ability to establish rapport while maintaining appropriate professional boundaries. Strong organizational, documentation, and time management skills. Proficiency with Microsoft Office Suite and electronic record systems. Valid Michigan Driver's License required.
