Speech Therapist PRN Weekday & Weekends

Job Summary Baylor Scott & White Medical Center - Sunnyvale is seeking a Speech Therapist to evaluate patients and develop speech language programs in collaboration with the physician and provide speech language therapy. The therapist instructs, motivates and assists patients in reaching therapy goals. Responsibilities Establish and evaluate treatment programs, plans of care, goals, and discharge plans for patients related to speech/language therapy needs in consultation with physicians. Revises plans and goals based on changes in inpatient status in consultation with physicians. Administers diagnostic speech and language evaluations. Assesses patient status and recommends, in writing to the patient’s physician, the need for a rehabilitation program, either restorative or maintenance. Interprets and implements physician orders and related services, ensuring compliance with ethics, patient rights, and state and federal codes. Coordinates the speech therapy program with other health care providers and disciplines. Completes and submits required documentation within agency guidelines. Facilitates effective communication among clinicians providing care. Demonstrates therapy for nurses, home health aides, family members, or other responsible persons who may assist with the patient’s rehabilitation program. Observes and evaluates the performance of those assisting in the speech program, providing guidance as necessary. Re-evaluates the patient’s progress and therapy program, making changes as needed and under physician direction. Participates in interdisciplinary patient care conferences and maintains ongoing communication with all disciplines involved. Contacts the physician with changes in patient status or program, obtaining new orders. Maintains accurate clinical records and charts changes in patient condition, adverse reactions, and any problems with informal supports or home environment. Interprets Medicare guidelines and obtains prior approvals as required by third‑party payers. Supervises and/or executes all skilled treatment programs as prescribed by the plan of care. Schedules patients with other team members. Instructs and informs RN of the patient’s medical status and speech therapy program; completes necessary documentation in the EHR. Integrates the work, efforts, and goals of occupational therapy with other team members. Provides client/family education as appropriate and documents it in the EHR. Qualifications Degree in Speech Language Pathology. Current state license/registration as a Speech Language Pathologist. One (1) year experience in Speech Language Pathology; prior acute‑care experience preferred. Successful completion of a competency and proficiency test and skills evaluation appropriate for the position upon hire. Pass a physical examination after being offered employment and a health assessment annually thereafter. Ability to lift up to 50 pounds, perform one‑person transfer, reposition bed‑bound patients, and perform the necessary physical tasks of the position. Certification in American Heart Association BLS. Proficient written and verbal English communication skills; additional languages preferred but not required. Strong presentation, computer, organizational, analytical and critical thinking skills.
