Behavioral Health Case Manager

Position Overview

Full-time Monday‑Friday 9am‑5pm. ViaQuest Psychiatric & Behavioral Solutions provides individualized care to children, adolescents, adults, and the aging population, including those with developmental disabilities or extreme trauma. A team of mental health counselors, social workers, certified nurse practitioners, case managers, and psychiatrists support clients for optimal outcomes.

Responsibilities

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordination of healthcare, behavioral health, non‑healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

Requirements

• Must be at least 18 years of age.
• Bachelor's or Master's level degree in a related field or a high school diploma with three or more years of relevant experience.
• Related experience or training with mental or behavioral health conditions.
• Valid driver's license, car insurance, and reliable transportation.
• Willingness to travel throughout assigned service area.
