The Home Health PT will make home visits in Hernando County (including Spring Hill, Weeki Wachee, Brooksville, and surrounding areas). What You’ll Do Provide physical therapy services to patients as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations. Perform initial and ongoing assessments, including OASIS assessments, to determine level of functioning and ability to perform activities of daily living. Develop and revise plans of care in consultation with the physician and other care team members to enable patients to achieve realistic mobility goals. Report to the physician and designated agency representative the patient’s reaction to treatment and any changes in the patient’s condition. What You’ll Need Licensure as a physical therapist in the state of Florida. Valid Florida driver’s license. Graduate of an approved PT program with an active Florida license. Home health experience is a plus. Preference for experience using Homecare Homebase (HCHB) EMR. Strong assessment, critical thinking, and patient education skills.
