RN Case Manager – Part‑time, weekend coverage for St. Ann’s Position Purpose Our RN Case Manager collaborates with internal and external disciplines to direct and coordinate patient care from admission through transfer or discharge, ensuring cost‑effective use of hospital resources, minimizing patient out‑of‑pocket expenses, providing customer support, and acting as patient advocate. Employment Type Part‑time Shift Saturday and Sunday, 8‑hour shifts What You Will Do Identify discharge needs, determine appropriate level of post‑discharge care, coordinate planning, and resolve continuity issues to prevent readmission. Ensure all clinical information is provided to outside agencies and communicate pertinent information professionally to the patient care team. Remove barriers to effective patient stay management and promote process improvements within the department, hospital, and community. Maintain professional development, licensure, and self‑education; advocate for patient legal, social, and ethical rights while balancing resources and hospital stewardship. Coordinate and facilitate patient/family education and refer quality or utilization issues to appropriate sources. What We Are Looking For Associate’s degree from an accredited RN program. Current license to practice as a registered nurse in Ohio. Minimum 3–5 years of recent clinical experience in an acute care setting. Availability to work Saturdays and Sundays, 8‑hour shifts.
