Key Responsibilities\n

  • Conduct thorough and ongoing assessments of patients to determine their needs and tailor treatment plans accordingly.

  • Collaborate effectively with supervisors, direct care staff, family members, and other stakeholders to communicate patient progress and necessary adjustments to care.
  • \n
  • Prepare tentative schedules for appointments, home visits, and progress observations, ensuring consistent monitoring of patient advancement.
  • \n
  • Organize and coordinate home visits for comprehensive assessments, which may include chart reviews and interviews with staff and patients.
  • \n
  • Ensure that treatment program plans are carried out effectively and adapt them to enhance patient outcomes.
  • \n
  • Participate actively in Interdisciplinary Team Meetings to discuss and strategize patient care and treatment methodologies.
  • \n
  • Create detailed monthly and quarterly progress notes to track patient development and clinical outcomes.
  • \n
  • Provide training to staff members to ensure competence in implementing program plans and maintaining accurate documentation.
  • \n
  • Conduct follow-up interviews, home visits, and observations to provide ongoing support according to individual patient needs.
  • \n
  • Maintain organized records that are easily accessible to authorized professionals and stakeholders.
  • \n
  • Act as a liaison to the broader community and mental health professionals to promote and support development goals.
  • \n
  • Contribute to the improvement of organizational practices by assisting in the creation of training materials and resources.

Patient Population / Client Focus\n

The counselor will have the opportunity to work with a diverse range of patients, including adults and youth facing various challenges. The core focus areas include family dynamics, substance misuse, trauma recovery, and overall mental health resilience. The LPC will play a critical role in guiding clients through their therapeutic journeys, promoting a culture of understanding and healing.

Qualifications\n

  • A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.

  • An active and unrestricted license as a Professional Counselor (LPC) in Colorado is mandatory.

  • A minimum of 2 years of counseling experience, particularly in clinical settings.
  • \n
  • Demonstrated specialization in areas such as family therapy, couples therapy, substance misuse, or trauma is highly desirable.

  • A strong understanding of legal and ethical standards relevant to counseling practice.

  • Outstanding interpersonal and communication skills with a proven ability to connect with a wide variety of clientele.

  • Proficiency in using Electronic Medical Record (EMR) software and other relevant technologies for effective record-keeping.

Compensation and Benefits\n

  • Annual salary range of USD 90,000 to 100,000, commensurate with experience and caseload availability.
