Clinical - LTSS Service Care ManagerImmediate need for a talented Clinical - LTSS Service Care Manager. This is a 06+months contract opportunity with long-term potential and is located in Pembroke Pines, FL, Hallandale, FL, Weston, FL, Dania, FL, Davie, FL (Onsite).Job ID:26-31942 Pay Range: $25 - $29/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).Key Responsibilities:Conduct assessments with members, caregivers, and providers to understand health status, support systems, functional needs, and required servicesHelp develop and maintain individualized long-term care and service plansCoordinate referrals, providers, specialists, and community resources to meet members' needsMonitor service delivery and care-plan outcomes, identify gaps in care, and recommend adjustments as neededMaintain regular contact with members through telephone calls and in-person visits, based on member needs and program requirementsCollaborate with physicians, discharge planners, care providers, caregivers, and other partners to coordinate acute and long-term careSupport members and families with information about available services, benefits, procedures, referrals, and healthcare optionsHelp coordinate informal or voluntary services that can be incorporated into members' care plansAssist members with filing and resolving complaints and appealsDocument assessments, member contacts, care-management activities, and outcomes accurately and in accordance with applicable requirementsIdentify opportunities to improve quality of care and service deliveryKey Requirements and Technology Experience:At least 2 years of case management experienceStrong Microsoft Office and computer skillsBilingual in Spanish with strong English skillsGood time management, organization, adaptability, and teamwork skillsAbility to participate in virtual training and work independentlyValid driver's license and ability to travel locally for field visitsBachelor's degree
