Job Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments while formulating personalized treatment strategies
• Implement evidence-based physical therapy techniques addressing mobility, strength, balance, and overall function
• Collaborate seamlessly with nurses, physicians, case managers, and other rehabilitation experts to enhance patient outcomes
• Educate patients and their families regarding therapeutic objectives, discharge steps, and prescribed home exercise programs
• Maintain comprehensive documentation of evaluations, treatments, and patient progress that adheres to facility and regulatory standards
• Engage in interdisciplinary team rounds and conferences for synchronized and coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (such as in neurology or geriatrics) are advantageous, but not required

Skills

• Prior experience is highly preferred
