Field Care Manager Nurse In-Home Behavioral Health

Humana Inc. is seeking a Field Care Manager Nurse 2 to assess and guide members toward resources supporting wellness in South Carolina. You will report to the Manager, Care Management of Behavioral Health and work to remove care barriers while monitoring progress toward outcomes. Required are a BSN with SC license, 2+ years in case/care management, and experience with behavioral health populations. Travel up to 50% is expected to support on-site and community interactions. #J-18808-Ljbffr
