Key Responsibilities of the Registered Nurse Case Manager

• Develop and revise treatment and discharge plans in collaboration with patients, families, and the care team
• Complete and submit OASIS assessments and necessary documentation in a timely manner
• Process orders and update physicians on patient status and changes
• Promote patient respect, privacy, and confidentiality
• Implement preventive and rehabilitative strategies
• Educate patients and families on disease processes and self-care techniques
• Supervise LPNs to ensure patient health and compliance with care plans
• Participate in the agency's operational and financial objectives
• Engage in professional development and diversity appreciation

Requirements of the Registered Nurse Case Manager

• Certified Home Health Care Experience Required
• EPIC or Netsmart experience preferred
• OASIS and Telemetry experience required
• BLS certification
• Current NY RN License
• One year of experience as a clinical nurse in a hospital required, 9 months home care experience preferred
• Strong critical thinking, assessment, and communication skills
• Current NYS driver's license
