Director of Patient Services (DPS) — Licensed Home Care

About the Role

The Director of Patient Services will oversee clinical operations, lead the clinical team, and partner closely with executive leadership to maintain high standards of patient care and regulatory compliance across the communities the organization serves.

Key responsibilities include:

• Oversee day-to-day clinical operations and patient care services
• Lead, mentor, and support clinical supervisors and field leadership
• Ensure compliance with New York State Department of Health (NYS DOH) regulations and maintain survey readiness
• Oversee quality assurance and performance improvement initiatives
• Review clinical documentation, patient care plans, and clinical workflows
• Identify opportunities to improve clinical processes, efficiency, and patient outcomes
• Support the growth and development of an established clinical team
• Partner with executive leadership on clinical and operational strategy
• Help shape clinical operations and patient care across the communities the organization serves
• Serve as a senior clinical resource for the organization
• Monitor clinical performance and ensure consistent adherence to policies, procedures, and standards of care

What We're Looking For

• Active New York RN license
• 5+ years of clinical leadership experience in home care
• Current or previous experience as a Director of Patient Services, Director of Nursing, Clinical Director, Director of Patient Care Services, RN Administrator , or comparable senior clinical leadership role
• Strong knowledge of NYS DOH regulations, healthcare compliance, quality assurance, and survey readiness
• Experience in LHCSA, CHHA, private duty nursing, or pediatric home care strongly preferred
• Proven ability to lead, mentor, and develop clinical teams
• Strong understanding of home care clinical operations
• Excellent communication, leadership, and organizational skills
• A track record of maintaining high standards of patient care and improving clinical processes
