Essential Qualifications

• Educational Background: A Bachelor’s, Master’s, or Doctoral Degree in Physical Therapy from an APTA-accredited institution.
• Licensing: An active Indiana PT license with no restrictions is mandatory to begin the assignment.
• Experience: A minimum of one year in physical therapy is required.
• Certification: Basic Life Support (BLS) certification is necessary (AHA or ARC accepted).
• Preferred Experience: Previous home health experience is advantageous, along with familiarity with HCHB and OASIS.

Assignment Overview

• Facility Type: Home Health.
• Shift Schedule: Day shifts, Monday through Friday, typically from 8:00 AM to 4:30 PM.
• Weekly Hours: Guaranteed 40 hours per week.
• On-call/Weekend Requirement: One weekend per month is expected.
• Coverage Area: Multiple counties including Clark, Floyd, Harrison, Jefferson, Scott, Washington, and Crawford, with travel between patient homes. Mileage will be reimbursed for travel from the first to the last patient.
• Key Responsibilities: Conduct regular home visits, manage start-of-care, resumption-of-care, and recertification visits; document in HCHB and complete OASIS assessments.
