POSITION OVERVIEW

Telos Health Systems is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA ) to serve as an in-house SNF-ist, providing comprehensive general medicine care to geriatric residents in a skilled nursing facility.

As an NP/PA with Telos, you will provide longitudinal care to short-term rehabilitation and long-term care residents while working closely with facility care teams to improve patient outcomes and reduce unnecessary hospitalizations.

WHAT YOU'LL DO

As an NP/PA SNF-ist, you will serve as a primary internal medicine provider for residents in the skilled nursing setting.

Responsibilities include:

• Manage the medical care of short-term rehabilitation and long-term care residents

• Perform comprehensive health assessments and physical examinations

• Diagnose and treat acute and chronic medical conditions

• Prescribe medications and treatments within your scope of practice

• Evaluate residents experiencing changes in condition

• Conduct routine and monthly stability visits

• Participate in advanced care planning and goals-of-care discussions

• Collaborate with nursing staff, medical directors, attending physicians, specialists, and interdisciplinary care teams

• Facilitate referrals to specialists and other healthcare providers as appropriate

• Maintain accurate clinical documentation and CPT coding

• Follow established clinical protocols and best practices

• Help identify opportunities to prevent avoidable emergency department visits and hospitalizations

WHAT YOU'LL NEED

Required:

• Active, unencumbered Nurse Practitioner (APRN/APN) or Physician Assistant (PA-C) license in North Carolina

• Current DEA registration

• Master's degree from an accredited Nurse Practitioner or Physician Assistant program

• Strong clinical judgment and communication skills

• Ability to work independently while collaborating effectively with facility-based care teams

• Strong organizational and documentation skills

Preferred:

• At least 1 year of experience in a Skilled Nursing Facility (SNF), long-term care, post-acute care, or similar setting

• Experience caring for geriatric or medically complex populations

• Experience managing acute and chronic conditions in a post-acute setting

FULL-TIME BENEFITS

Available to full-time employees working at least 30 hours per week. Benefits become effective following a 30-day eligibility period, on the first of the month thereafter.
