Qualifications Requirements: Physical Therapist license is required. Open to Physical Therapists at all experience levels. New grads are welcome. Key Responsibilities Screen and evaluate patients with cognitive and ADL functioning disorders; develop tailored plans of care. Utilize standardized assessments and evidence-based practices to inform clinical interventions. Provide rehabilitative treatments while adhering to regulatory standards. Administer rehabilitation procedures, including ADL retraining and therapeutic modalities. Assess patient progress and modify treatment plans as needed; document accordingly. Evaluate treatment outcomes and adjust services based on patient feedback, making referrals as necessary. Consult with patients, families, and caregivers regarding physical disorders. Communicate effectively with supervisors and the healthcare team about patient progress. Prepare comprehensive discharge summaries in compliance with regulatory requirements.
