Physical Therapist

Will mostly work outpatient Shift: 8a-430p Skills Required: Documentation: Assessment, Discharge, Evaluation, Long Term Goals, Plan of Care, Reassessments, Short Term Goals. Skills Preferred: Work Settings: Acute Care, Outpatient Clinic, Skilled Nursing Facility (SNF). Documentation: Time Management. Equipment and Devices: Bed alarms, Call lights, Crutches, Gait belt, Hospital bed, Hoyer lift, Lower extremity prosthetics, Rolling Walker, Shoulder slings, Sit to stand lift, Sliding boards, Standard Walker, Supplemental Oxygen, Traction table, Upper extremity prosthetics. Assessment: Body Mechanics, Pain assessment, Strength testing, Vital Signs. Specialty/Patient Population: Geriatrics, Orthopedics. Daily Census: 8 Number of Visits Per Day: 8 Number of Rooms/Beds: 25 Years of experience REQ: 1 Year. 2 Years is strongly preferred. Previous Charge Experience Preferred. First-timers accepted: No Weekend REQ: N/A National Certs REQ: (NBSTSA and/or NCCT) State Licensed BLS REQ: Preferred Is on-call REQ? N/A Will clinician float within scope to meet facility needs? Yes, within service line Floating Req to Sister Facilities: N/A Open to accommodating block schedule? No Locals accepted: No, home address must be at least 75 miles from the facility Pending State License accepted: N RTO Restrictions: 7 days Guaranteed Hours: 3 shifts per 13 weeks. The cancelled shifts must be the regular shift length to which the Contractor Personnel is assigned Special Requirements: Swingbed (Skilled) Care Inpatient Acute Outpatient. Type of Facility: Critical Access Hospital. Rehab department provides inpatient, swingbed (skilled) care and outpatient services.
