As a Social Services Associate, you will play a meaningful role in enriching the lives of older adults by guiding residents and families through rehabilitation, skilled nursing, and long-term care with compassion, dignity, and excellence.

Position Summary

The Social Services Associate serves as a vital member of the interdisciplinary care team, supporting residents and families from admission through discharge. This role coordinates psychosocial services, care planning, regulatory documentation, discharge planning, and community resource coordination while advocating for resident well-being and ensuring compliance with state and federal regulations.

Essential Responsibilities

• Coordinate admissions, psychosocial assessments, and required documentation for long-term care residents.
• Complete and maintain required MDS assessments, care plans, and social services documentation within regulatory timeframes.
• Participate in interdisciplinary care plan meetings, Medicare staffing meetings, and resident/family conferences.
• Conduct initial and ongoing family meetings to establish goals of care and develop individualized discharge plans.
• Coordinate safe and successful discharges by arranging physician orders, follow-up appointments, home health services, durable medical equipment, transportation, and community resources.
• Process PASRRs, ULTC-100 forms, PETIs, room changes, and other required state and facility documentation.
• Collaborate with nursing, therapy, physicians, and other departments to promote resident-centered care and smooth care transitions.
• Maintain resident confidentiality and ensure compliance with all CMS, state, and facility regulations.
• Promptly report suspected abuse, neglect, exploitation, or misappropriation according to facility policy.
• Participate in mandatory education, training, and other duties as assigned.

Qualifications

• Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred.
• Experience in skilled nursing, rehabilitation, long-term care, or senior living preferred.
• Working knowledge of MDS, discharge planning, Medicare/Medicaid processes, PASRR, and long-term care regulations is preferred.
• Strong organizational, communication, and interpersonal skills with the ability to manage multiple priorities.
• A compassionate, resident-centered approach and commitment to teamwork.
