The Clinical Care Coordinatorcoordinates patient discharge from hospital into hospice or palliative careprograms and performs start of care and initial nursing assessment. TheClinical Care Coordinator works cooperatively in coordination with the hospiceand/or palliative care teams. TheClinical Care Coordinator requires desire to work directly with patients andfamilies to assist them efficiently accessing hospice and/or palliative careservices. Must be able to utilize computers and becomfortable with electronic medical record. Must be able to meet with patients andfamilies in person and/or remotely QUALIFICATIONS: Graduate of accreditedschool of nursing with current nursing license in California. Certification in Hospiceand Palliative Nursing encouraged. At least one-year experience as a professionalnurse within the last 3 years Excellent verbal andwritten communication skills. Spanish speaking a plus,but not required. Valid driver’s license, and auto insurance, reliable and well-maintainedtransportation. #J-18808-Ljbffr
