Key duties include performing diagnostic evaluations, developing individualized treatment plans, and collaborating with primary care providers and other specialists to ensure continuity of care. The role involves both clinical oversight and direct patient interaction within an outpatient and/or inpatient setting as required by the practice model. Qualifications & Credentialing Education: Must be Board Certified or Board Eligible (if within five years of completing residency) in Neurology. Licensure: Candidates must hold an active Ohio medical license or be willing to obtain one. Certifications: A valid DEA certificate is required prior to the start date. Professional History: Must possess a clean malpractice and disciplinary record. Documentation: A recent NPDB Self-Query Report (completed within the last 30 days) is required at the time of submission.
