Employee Type Full Time Non-Exempt Tri-State Dialysis is a growing company based in Dubuque Iowa with locations in Dubuque IA, Manchester IA, Platteville WI and Richland Center WI. We are seeking a full-time Dialysis RN/LPN to join our team at our Richland Center, WI location. This position provides professional nursing care for patients in the dialysis unit and/or in the hospital. This position will be Full-Time (No Weekends) in clinic. Essential Duties: Essential job duties for this position include the following items. Other duties must be performed as assigned or required. Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes. Perform delegated independent nursing functions using established policies, procedures and standards as observed by the RN. Review patient’s records, clinician orders and patient’s condition, observe patient’s for changes in condition. Follow the treatment plan established for the assigned dialysis patient. Perform clinical procedures, including needle insertion and central line catheters, in accordance with established procedures and standards of care. Administer heparin according to patient’s established need or when necessary, take clotting time to establish more appropriate dosage of heparin. Administer medications and solution with accuracy and document response in medical record. Draw blood for laboratory specimens on a regular basis according to protocol of unit so that patient’s status may be evaluated. Complete Unusual Occurrence Form as necessary for defined situations and other situations as needed. Document appropriately to include double checks, labs drawn, medications given, patient assessment and condition of access, vital signs at current established time intervals, initiation and termination of treatment, condition of dialyzer at end of treatment and patient discharge summary following each patient dialysis treatment. Keep patients and dialysis equipment under constant observation during treatment, evaluating patients throughout and properly operating equipment. Report patient needs/problems and observations regarding patient’s condition to the RN and clinician. Document monthly on assigned primary patients, to include lab, anemia and bone disease flow sheets, Stable Patient Review form and nursing progress note. Update plan of care as needed. Attend monthly care conference on primary patients on assigned date. Provide patient/family education for promotion of optimal health, including information relating to the process of dialysis, function and care of access, fluid intake, diet and skin care. Identify the ability and willingness of patient/family to learn on continuous basis. Identify and utilize available resource personnel to facilitate delivery of quality patient care e.g. Renal Dietitian, Renal Social Worker, etc. Assist the Education Coordinator by acting as preceptor for new employees as assigned. Use personal protection safety equipment, including gloves, lab coats, gowns, and face masks/shields as appropriate. Maintain a clean, safe and efficient environment for patients, visitors and employees. Maintain infection control and safety guidelines, maintain equipment, instruments and stock supplies. Strive for professional and job‑related growth and development of self and others. Participate in corporate compliance program and follow HIPAA and HITECH guidelines. Comply with all policies and OSHA regulations related to safety, cleanliness, and infection control. Maintain confidentiality regarding Tri‑State Dialysis, its patients, and employees. Attend all mandatory programs, in‑services and staff meetings. Work toward continuous quality improvement and perform audits as assigned. Stay current with changing technology, including software programs and industry trends. Uphold, support, and promote all organization policies and procedures. Qualifications: To perform this job successfully, the Dialysis LPN/RN must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the minimum knowledge, skills and ability required. Education/Experience: Must be a registered RN or LPN Minimum one year of professional nursing experience preferred; Medical/surgical, hospital, chronic illness, critical care and/or community health experience preferred. Certificates, Licenses, Registrations: Must be a licensed LPN with an active license to practice in the state of Iowa and/or Wisconsin. Basic Life Support (BLS) Certification – required. Mandatory Reporter Certification – required. Will validate advanced specialized knowledge through certification by the appropriate organization.
