Client Care Coordinator

Client Care CoordinatorThe Client Care Coordinator is responsible for providing exceptional client care and support to our clients and their families. The clients served involve those with Intellectual and/or Developmental Disabilities ("I/DD") and IHC for Seniors and Disabled. This role requires traveling in and around the Florence/Coastal Area, excellent communication and organizational skills, as well as the ability to build and maintain positive relationships with clients and internal team members.ResponsibilitiesCoordinate and schedule client appointments and visitsEnsure accurate and timely documentation of client information and care plansRespond to client inquiries and concerns in a professional and timely mannerCollaborate with internal team members to ensure seamless client careAssist in the development and implementation of client care strategiesCoordinate with community partners to ensure seamless client careRequirementsHigh school diploma or equivalentPrior experience in client care, I/DD, IHC or a related field preferredExcellent communication and interpersonal skillsStrong organizational and time management abilitiesAbility to work independently and as part of a teamMust be able to travel in and around all of the Central Oregon areaBenefitsHourly Wage of $21-$23, paid every two weeksHealth Benefits (Medical, Vision, and Dental)Mileage ReimbursementPaid time offOngoing training and professional development opportunitiesAbout the Company:Almost Family, LLC is a leading provider of in-home healthcare services.
