Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.***Responsibilities:**The Social Work Manager is responsible for delivering high-quality services within the Assertive Community Treatment (ACT) programs. Working closely with the Program Director, they oversee clinical operations and provide both administrative and clinical supervision to the three ACT team leaders to ensure consistent service delivery. The manager also plays a key role in strategic planning, compliance with regulatory requirements, and maintaining SMH quality assurance standards. They ensure seamless coordination of care between ACT services and Strong Behavioral Health (SBH) Acute Hospital Services. This role involves overseeing the professional development of social workers across all ACT teams, providing clinical and disciplinary guidance. The Social Work Manager collaborates with leaders, including the Medical Director, Program Director, and ACT team leads, to maintain communication across disciplines. They offer training, consultation, and support in evidence-based therapeutic practices. Additionally, they work with the Program Director to ensure teams engage in quality improvement activities, presenting progress annually at Quality Steering Committee meetings. As the primary liaison with the Monroe County Office of Mental Health (MCOMH) Single Point of Access (SPOA), the manager ensures referrals are equitably reviewed and assigned to ACT teams based on need. They also collaborate with the Clinical Director of Strong Ties to support program development and service delivery for the Forensic ACT Team, ACT, and Project ACT, aligning with OMH guidelines. The Social Work Manager also provides direct patient care as needed, covering for ACT team leaders during absences, periods of high demand, or team leader vacancies, including on-call support.**ESSENTIAL FUNCTIONS*** As a member of the interdisciplinary leadership team for ACT program services, collaborates to develop and implement policies, procedures, and programs. Participates in the development and/or execution of quality improvement and outcome measurement initiatives in the service area. Provides administrative and clinical supervision to, Team Leaders, and clinicians (both social work and other disciplines) consistent with the program and department standards. Provide supervision to the newly formed role of the IPS (Individual Placement and Support) Specialist, who will work across all ACT teams, per ACT Guidelines to support employment goals for patients on ACT. Help to develop ACT-specific documentation templates to assist staff in meeting documentation requirements. Supports the development of the new Forensic ACT team, including implementation of any new specific OMH Guidance for Forensic Teams.* Attend and/or facilitate weekly treatment team meetings, for each team, as well as other general staff/therapist meetings. Attends monthly ACT team leaders meeting with SPOA team for Monroe County.* Consults with other members of the treatment team on the identification,* assessment, and reporting of child abuse and neglect or domestic violence.* Participates in departmental clinical case reviews, including Patient Care Coordination meeting, for patient who are high risk or high utilizers of services. Attends and can participate in organizing any community meetings to coordinate care for patient with service providers across agencies.* Supports in recruitment of all disciplines, across all teams to ensure minimal disruption in care due to staff fluctuations. Support and create orientation/onboarding plan for all new hires. Supports the evaluation of performance of the ACT team lead, social workers, and any other disciplines as appropriate, including interviewing, quarterly audits of the electronic medical record, and yearly performance evaluations. In collaboration with the Program Director, and with HR, works to address any performance issues and any other disciplinary actions, as indicated. Collaborates with program director to ensure professional development of all staff.* Provide operational supervision to ACT team Program OAS to ensure patient care needs and workflows for team members are efficient. Orientation for new OAS’s. Ensure workflows are efficient and maximize support to ACT Team Leaders and their teams.* Provide oversight of the P-Card utilization, authorizations for P-Card utilization, ensure timely approval of SOE’s, and petty cash.* Reconcile monthly billing spreadsheet after completion by OAS and before submission to Department Billing Manager for monthly charge entry for ACT services.* Time Reporting/Payroll for ACT staff. Review and approve time off requests.* Attends Division, Department, and Clinic level staff meetings. Ensures that Division, Department, and Clinic policies and procedures are implemented with regard to professional practice and patient care. Ensures timely completion of statistical reports/forms, time sheets, and other Hospital or Division mandatory in-service trainings and Health Assessments. Subscribes to maintaining a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competency events and training appropriate to job duties.* Completes Incident Reviews, according to URMC policy, and chart reviews for ACT Team incidents that are reported to Justice Center/OMH. Work with Department ACT Utilization Review team to address documentation concerns. Attending monthly review meetings and quarterly reviews with each team. Works on any educational or procedural changes needed to meet utilization review standards. Leads annual fidelity assessments for each ACT team, per OMH regulatory standards. Implement any workflow changes related to any Incident Review findings. Document any changes, corrective action plans, or follow-up.* With the support of the Strong Ties Program Director implement and support activities required to prepare for regulatory surveys from JCAHO and OMH. This includes, but is not limited to, ensuring implementation of survey findings, performance improvement plans, and environment of care, and patient care surveys to ensure findings are utilized to improve patient experience. Assist with Assisted Outpatient Treatment (AOT) Service Verification surveys that occur quarterly to ensure the teams are meeting AOT specified treatment interventions. Correspond with Western NY OMH representative and Monroe County AOT coordinator regarding any deficiencies and ensure timely follow-up by staff.* Documents interventions, both direct and indirect, in the patient’s medical record in compliance with the Division, Department, and Hospital policy.
