Maniilaq Association isa P.L. 93-638 Native/Indian Preference/EEO Employer Case Management Support I / II Job Code: 2026:OTZ-149 Location: Kotzebue, Alaska Program: Outpatient Services FT/PT Status: Full Time Job Responsibilities: Maniilaq Association 4/26 Title: Case Management Support I/II Range: TBD Program: Outpatient Services Status: Non-Exempt Housing Priority: None Covered: Yes POSITION SUMMARY Responsible for providing administrative support for the Nursing Case Manager and Medical Providers assigned Care Team. Responsible for assisting patients with questions, appointments, referrals, etc. Reports to the Outpatient Services Manager. PRINCIPAL DUTIES AND RESPONSIBILITIES Carries out reception duties including receiving, screening, and forwarding incoming phone calls, and prioritizing and passing on information. Performs clerical duties as required to support assigned Care Team, including typing letters, memos, minutes, reports, faxing and copying. Maintains files in a secure and organized manner follows HIPPA. Compiles and inputs data into computer system as directed by the Care Team. Notifies patients of normal labs only when directed to do so by provider or nurse. Copy information and distribute as indicated by Medical Providers and Care Team. Assists with Care Team Projects as assigned including patient databases and pending follow-up. Assists with the referral process, including patient travel, in collaboration with Specialty and Care Team staff as directed. Assists Care Team with patient letters and travel worksheets as directed. Participates in departmental and organizational performance improvement activities. Treats all patients and staff with dignity and respect. Schedule appointments at MHC as directed by the Care Team. Proficient in use in Cerner or current Electronic Health Record. Knowledgeable of appropriate information from the patient record that should be included with referrals. Manage assigned queues/pools. Perform online cardiac event monitor registrations when ordered by providers. Works, prints, and forwards appropriate teams the AFHCAN queues as assigned. Collaborates with appropriate case manager concerning problems in referring the patient. Assists as needed in making follow up appointments or getting request to the schedulers. Assists the Telerad x-rays for radiology as directed by the nurse or provider. Provides good telephone and customer service skills. Responsible for dealing with all scheduling queries from patients and hospital administration. Copy information and distributes to Outpatient Program staff. Other duties as assigned. Job Qualifications: MINIMUM REQUIREMENTS Case Management Support I : High school diploma or equivalent plus 6 months previous health service experience preferred. Must be willing to complete Medical Terminology course and receive certification within 12 months of hire. Knowledge of Cerner EHR system preferred. Ability to utilize various computer programs and type well enough to carry out job duties efficiently. Must be able to maintain strict confidentiality following HIPPA at all times. Requires good interpersonal and communication skills. Requires strong organizational skills and ability to maintain file systems. Requires excellent customer service skills and telephone etiquette. Cerner and Revenue training required. Medicaid travel training required. AFHCAN training required. MHC Travel department observation training. Able to utilize tiger text to communicate with MHC and ANMC teams. Case Management Support II : In addition to Case Management Support I requirements: Medical Terminology certification required. 6 months of Cerner or current EHR experience required. Maintains professionalism with coworkers, staff, and patients. Requires exceptional attendance able to work independently and efficiently. Attend weekly team meeting with case managers, providers, CHAP and village clinic staff. Able to process all referrals. Able to cover all areas as needed including the appointment desk, or other case management support positions. Able to schedule all MHC appointment types. Able to schedule village appointments and send appropriate travel information.
