Home Health RN Case Manager – Fort Worth / Tarrant County, TX

We are in search of a skilled Home Health RN Case Manager to work in the field in Fort Worth, TX and the nearby Tarrant County area. This role requires a Registered Nurse with strong clinical abilities, recent experience in home health case management, excellent assessment skills, thorough documentation practices, and the capability to independently manage a caseload of patients in the field.

Hours: Full-time

Experience: Recent experience in Home Health RN Case Management

Main Duties

• Oversee a caseload of home health patients in the field.

• Conduct skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.

• Create, revise, and coordinate personalized care plans with input from physicians, patients, families, and the interdisciplinary team.

• Maintain accurate and timely clinical records, including OASIS and visit notes.

• Monitor patient progress, identify changes in condition, and communicate updates as needed.

• Educate patients and caregivers on disease management, medications, safety, and care plan objectives.

• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.

• Adhere to home health regulations, documentation requirements, and quality standards.

Requirements

• Active Texas RN license in good standing.

• Recent experience in Home Health RN Case Management required.

• Proficiency in Medicare home health, OASIS, care plan management, and field-based nursing.

• Ability to work independently while maintaining effective communication with the clinical team.

• Strong clinical judgment, organizational skills, documentation practices, and time management abilities.

• Valid driver's license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County.

• Experience with EMR systems is preferred.
