Key Responsibilities\n- \n
- Conduct comprehensive psychosocial assessments through interviews, clinical observations, and medical record reviews to gauge the emotional, social, and environmental factors impacting patients' recovery. \n
- Provide empathetic counseling and emotional support to students and their families, addressing concerns related to their rehabilitation journey and emotional adjustment. \n
- Collaborate with the rehabilitation team—including physical therapists, occupational therapists, and speech-language pathologists—to develop individualized treatment plans that reflect the unique psychosocial needs of each patient. \n
- Assist clients in accessing community resources such as support groups, vocational rehabilitation opportunities, educational resources, and housing assistance to help them transition back into their communities successfully. \n
- Offer guidance and education to families regarding available community resources and post-rehabilitation support services to empower them during the recovery process. \n
- Participate in discharge planning processes to facilitate smooth transitions for patients leaving the rehabilitation setting, ensuring continuity of care. \n
Your work will primarily focus on school-aged children and adolescents recovering from various conditions, including physical injuries, neurological disorders, and emotional challenges. You will be instrumental in creating a supportive environment that fosters resilience and encourages students to overcome barriers on their path to recovery. By addressing the unique needs of this population, you will play a critical role in promoting their emotional and psychological well-being during a pivotal time in their lives.
Qualifications\n- \n
- Must possess a valid and active license as a Clinical Social Worker (LCSW) in Pennsylvania. \n
- A Bachelor’s degree in Social Work or a related field is required; a Master’s degree in Social Work is preferred. \n
- Minimum of 1 year of clinical experience in a health care setting, preferably in rehabilitation or school-based environments. \n
