You’ll collaborate with physicians, interdisciplinary teams, and community health professionals to ensure seamless, compassionate care.Key Responsibilities:Assess and evaluate patient statusDevelop and revise individualized care plansProvide palliative nursing care and educationAct as Case Manager when assignedMaintain accurate clinical documentationCommunicate with physicians and community partnersQualificationsHomecare Homebase/Point Care EMR experience is an added bonus.This role is ideal for someone who thrives in a patient-centered environment and values meaningful connections.
