Physical Therapist

Physical TherapistWe are seeking a dedicated and compassionate Physical Therapist to join our interdisciplinary team in supporting individuals with developmental and physical disabilities. In this role, you will provide essential therapy services that enhance mobility, promote independence, and improve overall quality of life. This contract is ideal for a therapist with experience in developmental disabilities across age groups, with a preference for those familiar with pediatric care.Responsibilities:Develop and implement individualized habilitation plans (IHPs) to address residents' gross motor skills, mobility goals, and adaptive equipment needs.Conduct thorough physical therapy assessments upon referral and deliver treatment sessions in various settings, including therapy rooms, residential homes, and day programs.Document all therapy interactions, evaluations, and treatment progress in accordance with organizational and regulatory standards.Provide physical therapy summary reports aligned with each resident's transition timeline.Participate in interdisciplinary IHP conferences and collaborate in care planning to ensure cohesive and person-centered treatment.Evaluate and recommend adaptive equipment and therapy tools to enhance residents' independence and functional abilities.Schedule and provide therapy services within the contracted hours, including evenings and weekends when necessary.Ensure evaluations are conducted per age-specific guidelines: Annually for residents aged 10 and underEvery two years for residents aged 11–22 (unless otherwise requested)Every three years for residents aged 23 and olderSubmit evaluation results at least five days prior to the resident's IHP meeting.Engage families in treatment planning and education when appropriate, supporting a holistic and inclusive approach to care.Maintain regular, collaborative communication with interdisciplinary team members, including physicians, nurses, therapists, psychologists, QIDPs, HCS staff, dietitians, social workers, and day program staff.Qualifications:Licensed Physical Therapist in the state of Ohio (required)AHA BLS certification (required)Experience working with individuals with developmental disabilities and/or pediatrics (preferred)Strong documentation and communication skillsAbility to work both independently and as part of a multidisciplinary teamFlexibility to work non-standard hours, including evenings and weekendsBenefits:Competitive salary and benefits package.Opportunities for professional growth and advancement.Access to ongoing training and development programs.Supportive and collaborative work environment.Meaningful work that makes a difference in patients' lives.Alto Health Care Staffing is an equal opportunity employer that is committed to diversity and inclusion in the workplace.
