Job Posting: Home Care Coordinator Position: Home Care Coordinator Type: Full-Time Schedule: Monday - Thursday 8:00 AM - 4:00 PM Friday, 8:00 AM - 1:00 PM Location: Woodcliff lakes, NJ About Us: At CareSphere, we are dedicated to providing exceptional home care services that enrich the lives of our clients and support their well‑being in the comfort of their own homes. We are seeking a compassionate and detail‑oriented Home Care Coordinator to join our team and help us deliver high‑quality care. Key Responsibilities: Oversee and coordinate home care services for clients, ensuring their needs are met with the highest standard of care. Develop and maintain care plans in collaboration with clients, families, and healthcare providers. Schedule and manage caregiver assignments, ensuring appropriate match between caregivers and clients. Monitor client progress and adjust care plans as needed. Address client and caregiver concerns and resolve issues in a timely and professional manner. Maintain accurate and up‑to‑date records of care services provided. Ensure compliance with all relevant regulations and company policies. Work efficiently in a fast‑paced environment, managing multiple tasks and priorities effectively. Qualifications: High school diploma or equivalent required; Associate’s or Bachelor’s degree in healthcare, social work, or a related field a plus. Previous experience in home care coordination or a related field is highly desirable. Strong organizational skills with a keen attention to detail. Ability to work quickly and effectively under pressure. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Proficiency in Microsoft Office and other relevant software. Must be able to work Mon‑Fri 8am-4pm in Suffern office.
