LTSS Service Care Manager

Location: Remote and face-to-face visits to members in the Texas (Dallas, Nueces, and Central SDAs areas. *Travel days focus on completing 2-3 in-person assessments daily (~75% travel), while doc days focus on completing required documentation within 3-day TAT, care planning, and follow-ups


Shift: Mon - Fri 8am - 5 PM CST


Duration: 12 months


Target start date: Aug 31, 2026


Description: -Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs.


Typical day in the role: SCs manage member cases through outreach, face-to-face visits, and care coordination. The role includes significant fieldwork (about 75% travel), completing assessments, coordinating services, and ensuring members receive needed care while maintaining documentation and compliance standards.
