Job Summary: Dennett Rehab Center is seeking a dedicated and experienced MDS RN to join our team of healthcare professionals. As an MDS RN, you will be responsible for leading, coordinating, and overseeing the full collaborative, interdisciplinary assessment and care planning process, including comprehensive resident assessments, care coordination and planning, resident advocacy, collection and transmission of data for quality improvement and finances, and adherence to the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) requirements. Job Duties: Participate, schedule, and oversee scheduling of all resident MDS assessments to ensure timely completions by the interdisciplinary team Coordinate and oversee all functions of the MDS assessment process to ensure compliance with federal and state requirements Provide leadership for care planning and complete care plans as needed in collaboration with nursing leadership and staff nurses Provide leadership of care planning meetings with residents' and families/loved ones and follow through with planning and/or communication of identified needs for resident care Participate in the pre‑admission process to determine potential residents' level of care and/or to assist with decisions regarding admissions Obtain medical eligibility for residents applying for Medical Assistance Stay aware of clinical and service excellence best practices in rehabilitative and extended care nursing and coach members of the nursing care team to success regarding best practices Why Work for Us: Competitive salary Comprehensive benefits package, including paid time off, 401k plan, health insurance, dental insurance, and vision insurance Opportunities for professional growth and development Qualifications: Graduate of an accredited school of nursing; RN Valid RN license in Maryland Experience in a long‑term care environment preferred Experience with the MDS/RAI process and/or case management preferred If you are passionate about delivering exceptional care and services, and are looking for a rewarding career opportunity, we encourage you to apply for this MDS RN position at Dennett Rehab Center.
