RN - Case Manager

Case ManagerThe Case Manager plays a crucial role in the coordination and management of patient care. This position involves working directly with patients to assess their needs, develop care plans, and facilitate necessary services across all areas of the facility. The ideal candidate will have strong organizational skills and the ability to collaborate effectively within a multidisciplinary team.Key ResponsibilitiesConduct thorough initial assessments and develop and implement care plans for patients.Perform utilization reviews and ensure compliance with payers and regulations.Coordinate discharge planning to facilitate smooth transitions for patients.Engage with a diverse patient population, including various payers, uninsured individuals, and special needs cases.Document all patient interactions and care plans using electronic medical records (EMR), specifically Interqual.Collaborate with healthcare team members, including social workers and healthcare providers, to achieve optimal patient outcomes.Adjust priorities and tasks throughout the day based on patient needs and urgent situations.QualificationsMinimum of 2 years of case management experience required.BLS Certification is mandatory.Familiarity with Interqual criteria is preferred.Strong communication and teamwork skills, with the ability to work effectively in a dynamic environment.Experience in managing care for diverse populations, including the uninsured and homeless individuals.Work ScheduleThis position is for full-time employees working 40 hours per week. Shifts are typically 8 hours long and may include weekend shifts on a rotating basis, with at least one full weekend per month. While holiday work is not mandated, willingness to work on holidays is appreciated.Orientation and TrainingThe onboarding process includes a comprehensive 5-day orientation specific to care coordination, with unit-specific training tailored to the individual's experience and background.Additional InformationThe Case Manager will have the opportunity to build their skills in both Utilization Management and Discharge Planning, which are essential components of this role.
