Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings, crafting individualized treatment strategies
• Implement physical therapy interventions centered on mobility, strength, balance, and function improvement
• Collaborate with a multidisciplinary team of nurses, physicians, case managers, and rehabilitation professionals to enhance patient outcomes
• Educate patients and their families about therapeutic goals, discharge planning, and prescribed home exercise regimens
• Maintain accurate documentation of evaluations, treatments, and progress in keeping with facility standards and regulations
• Engage in care team discussions and interdisciplinary meetings to ensure cohesive patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• Preferred: CPR certification
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory

• Previous experience in the field is strongly preferred
