Responsibilities

• Conduct comprehensive PT evaluations and develop individualized treatment plans for residents
• Implement and adjust therapy programs to improve mobility, strength, balance, and functional outcomes
• Monitor progress, document clinical findings, and communicate recommendations to the care team
• Provide patient and family education on exercises, safety, and plans for discharge or transition
• Ensure safety and infection control standards during therapy sessions
• Collaborate with a multidisciplinary team to coordinate care and optimize rehabilitation results

Requirements

• Active, unrestricted Physical Therapist license in New Hampshire or eligible for licensure in NH
• Experience in physical therapy, with prefer­ence for skilled nursing facility or geriatric rehabilitation settings
• Strong clinical assessment, treatment planning, documentation, and communication skills
• Ability to work independently within a multidisciplinary team and manage a caseload
• Availability to start July 13, 2026 and commit to a 13-week travel assignment with guaranteed hours
