Long-Term Care Pharmacist – Evening Shift

Location: San Antonio, TX | Full-Time | On-Site
Schedule: Monday–Friday, 1:30 PM–10:00 PM
Weekends: No regular weekend shifts
On-Call: Approximately once every 2 months

Position Overview

We are seeking a dedicated and experienced Long-Term Care Pharmacist to provide accurate, timely, and patient-centered pharmacy services to long-term care and skilled nursing residents.

This is an excellent opportunity for a clinically focused pharmacist seeking a predictable weekday evening schedule with limited on-call responsibility .

Key Responsibilities

• Review, verify, and dispense medications accurately.
• Evaluate orders for interactions, allergies, duplications, contraindications, and other medication concerns.
• Provide clinical oversight and medication recommendations.
• Process new orders, changes, refills, and urgent requests.
• Monitor medication therapy and promote resident safety and outcomes.
• Respond to medication-related questions from nurses and prescribers.
• Maintain compliance with pharmacy laws, regulations, and organizational policies.
• Maintain accurate records and documentation.
• Participate in quality assurance, medication safety, and continuous improvement.
• Support efficient pharmacy workflow and provide on-call/backup support as scheduled.

Qualifications

• Current, unrestricted pharmacist license in the applicable state.
• PharmD or equivalent pharmacy degree from an accredited program.
• Long-term care, institutional, hospital, or clinical pharmacy experience preferred.
• Strong knowledge of medication therapy, drug interactions, and pharmacy regulations.
• Excellent attention to detail, communication, and clinical decision-making skills.
• Ability to work independently and collaborate effectively with healthcare professionals.
• Ability to work efficiently without compromising accuracy or patient safety.
• Reliable, professional, and patient-focused.
