Mobile Case Manager: Mental Health & Housing Stabilization

St. Joseph Center is seeking a full-time Case Manager to work with homeless individuals in California. The role involves conducting mobile outreach, linking clients to housing resources, and continuing support once they are housed. The ideal candidate will have a Bachelor's degree in social work or related field, and strong knowledge of homeless services. Responsibilities include developing client treatment plans, providing ongoing assessments, and facilitating access to a variety of needed services.
