As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APCs, registered dietitians, nurses, and pharmacists to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity‑related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health. Schedule: Monday–Friday with flexible scheduling, 32 patient contact hours minimum (+8 hours of administrative time). Hybrid model of care. No scheduled evening or weekend responsibilities outside of phone on‑call. Location: Midtown Manhattan EMR: Athena On‑Call: Phone call only, rotation TBD frequency. Call volume – very low 24/7/365 physician backup. Requirements Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA). Unrestricted DEA licensure and prescriptive authority (per state practice regulation). At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred. If no formal training in weight management, willingness to participate in knownwell clinical education and training plan for weight management and/or pursue OMA APC certification. Active, unrestricted license to practice in NY, or willingness to obtain. Willingness to obtain additional licensures and pursue independent practice if needed based on business needs. Excellent communication and interpersonal skills. Dedicating to staying current with latest developments in primary care and obesity medicine. Believes in inclusive, compassionate care. Believes in shared decision making with a patient; honors the goals a patient has for their health and partners with them to achieve those goals. High emotional intelligence and knows how to make a patient feel at ease—without judgment and without shame. Willing and excited to try new technologies that are both patient and provider oriented. Excited to work with a collaborative team of physicians, APCs, registered dietitians, medical assistants, health coaches, and other staff. No visa sponsorship available; applicant must be authorized to work in the United States. Responsibilities Practice exceptional medicine within knownwell. See adults for primary care, often with a focus on metabolic health, or adults for metabolic health only. Share a panel with MD/DO and other APCs. Embodies the knownwell mission and values of patient inclusivity in care delivery. Coordinates with the broader care team to ensure patients have the support they need. Documents as necessary to ensure accurate billing, including review of remote patient monitoring data. Communicates with patients in a timely manner through our portal for questions and concerns that require a clinician response. See adults ages 16+ for metabolic health and primary care. Ensure panel is open to any individual, regardless of current health status or comorbidities. Listen to patients’ aspirations and ensure care is tailored to each unique health journey. Provide an environment in which patients feel comfortable and free from judgment. Offer exceptional encouragement as patients reach their goals. Coordinate across the collaborative care team. Escalate concerns to dedicated site leadership team. Provide clinical guidance to medical assistants and nursing teams as assigned. Stay current with licensure and appropriate re‑certifications. $150,000 - $170,000 a year.
