GENERAL RESPONSIBILITIES
The Program Director, Community Prevention and Treatment Services (CPTS) is responsible for the development and monitoring of systems that support the fiscal management, program planning, implementation, monitoring, supervision, and evaluation of all activities related to HIV and Hepatitis C (HCV) treatment, prevention and outreach programs. The Program Director provides active and on‑time collaboration with the clinical staff with specific responsibility for the provision of HIV and HCV services. The Director functions as WYH’s representative on HIV and HCV issues at local, state and federal forums and with funders and regulators of HIV and HCV services. The Director participates in the writing of grants related to maintaining existing programs as well as obtaining additional funding opportunities.LEADERSHIP/ADMINISTRATIVE RESPONSIBILITIES
Provides leadership that will support the principles of the Patient Centered Medical Home model of care for CPTS patients. Develops and supports an environment that is conducive to providing growth and development for employees. Ensures that the policies and procedures relative to the provision of HIV/Hepatitis C services meet the standard of care and are consistent with the goals and objectives of the organization. Provides leadership and linkages to and from the staff within the CPTS department, other departments, and management staff. Provides supervision and mentorship to staff and interns. Investigates problems and suggests solutions to the community regarding public health issues, presented by patients who are utilizing CPTS services. Facilitates and leads regular staff meetings. Demonstrates an understanding of cultural competency with emphasis on its application to impact and sustain changes that promote excellent healthcare.OPERATIONS MANAGEMENT
Ensures the registration process reduces barriers to access to HIV/HCV care and services meeting the needs of existing as well as new patients entering the system. Oversees the day‑to‑day operations of CPTS including the flow of patients throughout WYH. Ensures that the program(s) are adequately supplied with materials to meet the overall established HIV standard of care. Ensures that the operation and processes for HIV/HCV counseling and testing are adequate to meet the needs of patients and all funding sources. Collaborates with the Director of Behavioral Health to create seamless workflows—ensuring linkages to Collaborative Care and BH Clinic services for CPTS patients in need of mental health, addiction, and/or co‑occurring treatment services. Participates in weekly Case Conferences. Facilitates the development and maintenance of an effective Client Advisory Board. Conducts regular observation and review of grant‑funded staff activities, assuring the effectiveness and fidelity of the implementation of interventions outlined in grant work plans.PROGRAM DEVELOPMENT AND GRANTS MANAGEMENT
Conducts market analysis relative to HIV/HCV funding based on trends in the HIV/HCV community with consideration of state and federal initiatives with emphasis on the disease model. Initiates research of new funding sources for program enhancement and expansion. Prepares and submits grants to probable funding sources to enhance existing program, or to develop new programs. Serves as the Principal Investigator/Project Director for major grants awarded to WYH. Evaluates progress toward goals and objectives as described in work plans for funded grants that support the HIV/HCV programs. Provides timely reports for both internal and external stakeholders. Provides required program reports to funders of CPTS programs.FISCAL RESPONSIBILITIES
Approves all departmental purchase orders, travel requests, and requisitions. Assists with the preparation of annual budgets and ensures appropriate spending patterns. Assists with the development of a system that will ensure that all revenue generated and billed is collected. Provides supervision for the processing of all unit time and attendance records. Provides input to staff regarding the budget expectations including cost containment, revenue maximization, and productivity. PERFORMANCE IMPROVEMENT / PATIENT SAFETY ACTIVITIES AND RISK MANAGEMENT Integrates and articulates the institutional performance improvement activities into the CPTS program services, demonstrating outcomes from the PIPS CPTS activities to external parties. Coordinates quality improvement and program evaluation activities in accordance with requirements of funding agencies and organizational needs. Conducts unit‑specific performance improvement activities relative to work plan submitted to funding sources and to meet organizational needs. Demonstrates an understanding of unit‑specific and organizational risk management relative to care and services that are provided from and within the unit.MISCELLANEOUS
Demonstrates excellence in both internal and external customer service. Understands and is able to effectively communicate HIPAA compliance, corporate compliance and client confidentiality. Ensures and/or remains in compliance with local, state, and federal regulation, i.e. DHHS HRSA and NYSDOH, and all accreditation standards (e.g. Joint Commission and NCQA‑PCMH). Adheres to the National Patient Safety Goals as defined by the Joint Commission and Whitney M. Young Jr. Health Center. Completes other duties as assigned.REQUIREMENTS
Graduate degree in Social Work, Health Administration, Public Health or an allied health field. At least five (5) years of professional experience in a health‑related facility. A minimum of three (3) years functioning in a supervisory/management capacity. Comprehensive knowledge of grant writing techniques, budgeting and program management. Prior experience using an electronic health record. Demonstrates an understanding of the HIV and HCV disease treatment and prevention strategies. Must demonstrate excellence in oral and written communication skills and analytical ability. Possesses an understanding of the impact of the social determinants of health on patient engagement and outcomes.PREFERRED QUALIFICATIONS
Preference for a licensed clinician in a related clinical discipline with a graduate degree. At least three (3) years’ of management experience in a healthcare setting, preferably with HIV/HCV services. Experience as a program manager. Previous grant writing skills, budgeting experience and profit/loss responsibility.
