Case Management Nurse

We are seeking a compassionate, organized, and self-directed RN Case Manager to coordinate and monitor community-based services for older adults and individuals with disabilities throughout Rhode Island. This is a hybrid, community-based position that requires regular local travel. RN Case Managers are responsible for meeting with assigned members face-to-face in their homes or at mutually agreed-upon community locations at least once every six months, with additional in-person visits as needed based on member needs, changes in condition, or program requirements. Reliable transportation and the ability to travel throughout assigned service areas are essential to this role. The RN Case Manager serves as the primary point of contact for assigned members, families, service providers, caregivers, and community partners. This role focuses on ensuring members have the services, supports, and resources needed to achieve their goals while promoting independence, continuity of care, and quality outcomes. Key Responsibilities Care Coordination & Case Management Manage an assigned caseload of approximately 75 members. Conduct required monthly member contacts by telephone or other approved communication methods. Complete face-to-face member visits at least every six months , traveling to members' homes or agreed-upon community locations. Conduct additional in-person visits when needed based on member needs, changes in health or circumstances, or program requirements. Monitor member health status, service utilization, progress, and identified goals. Coordinate services with providers, caregivers, family members, community agencies, and other stakeholders. Identify changes in member needs and facilitate appropriate adjustments to services and supports. Develop and maintain annual person-centered plans in collaboration with members and their support systems. Maintain timely, accurate documentation and ensure compliance with applicable regulations, program requirements, and organizational policies. Required Qualifications Current, unrestricted Registered Nurse (RN) license . Knowledge of care coordination, case management, or community-based services. Strong communication, organizational, time-management, and documentation skills. Proficiency with electronic documentation systems and Microsoft Office applications. Reliable transportation and the ability and willingness to travel to member homes and community locations throughout assigned areas in Rhode Island. Experience working with older adults or individuals with chronic conditions, disabilities, behavioral health needs, or complex care needs. Previous case management, care coordination, home health, or community-based nursing experience. Work Environment and Travel Hybrid position combining remote/office work with field-based member visits. Local travel is a required and ongoing part of this position. In-person visits are required at least every six months for each assigned member and may occur more frequently depending on individual member needs. Visits may take place in a member's home or another identified community location. Flexible scheduling may be necessary to accommodate member and program needs.
