Hospice RN Case Manager

Addus Family Company is seeking a Hospice RN Case Manager to provide compassionate, in-home hospice care to patients in the Hillsboro area. In this role, you will manage a caseload of hospice patients, complete comprehensive assessments, develop and update individualized care plans, and coordinate services with physicians, social workers, aides, and family caregivers. You will provide skilled nursing care, symptom and pain management, patient and family education, and timely documentation, while ensuring compliance with hospice regulations and quality standards in a mission-driven, patient-focused environment.
