CNA - LTC/SNF

Job Summary:
Provides essential personal care and supportive services to residents in a long-term care or skilled nursing facility. Assists nursing staff with daily activities and helps ensure residents' comfort and well-being.

Responsibilities:

• Assist residents with activities of daily living in accordance with individualized care plans.
• Support residents with lifting, turning, repositioning, and transferring to and from beds, chairs, and wheelchairs.
• Aid in meal service, including setup, feeding assistance, and documenting meal intake.
• Deliver nutritional supplements and ensure they are consumed as prescribed, recording intake carefully.
• Monitor and document residents' weight, skin condition, and any changes in health status.
• Report concerns or changes in residents’ condition to nursing staff and document accordingly.

Qualifications:

• Minimum of 1 year experience in long-term care or skilled nursing facilities.
• High school diploma or equivalent required.
• Valid BLS certification (AHA).
• Successful employment history with durations longer than 13 weeks at previous roles.
• No disciplinary actions against current certification.
• Active state license or certification as a CNA.
