Senior HHA - Shift Supervisor
DEPARTMENT: Administration POSITION: Senior HHA - Shift Supervisor SUPERVISOR: RN
• Serve as the primary staff leader and point of contact during the assigned shift.
• Coordinate assignments and help ensure appropriate coverage of resident-care and operational responsibilities.
• Conduct shift huddles and communicate assignments, priorities, resident concerns, and relevant updates.
• Monitor staff attendance, punctuality, appearance, conduct, and completion of assigned duties.
• Verify that resident-care assignments, personal-care logs, medication observation records, and other required documentation are completed accurately and on time.
• Make regular rounds throughout the building and resident-care areas.
• Monitor staff interactions with residents, families, visitors, and coworkers to ensure professional and respectful conduct.
• Enforce organizational policies, including policies governing personal cell phones, break locations, confidentiality, resident rights, workplace conduct, and use of common areas.
• Redirect staff immediately when policies, care plans, or performance expectations are not being followed.
• Document and report policy violations, performance concerns, resident-care issues, incidents, and unusual occurrences to the RN or appropriate administrator.
• Promptly notify the RN of changes in a resident's physical, mental, emotional, or behavioral condition.
• Respond to resident and family questions or concerns professionally and refer clinical or administrative matters to the appropriate person.
• Assist with managing callouts and coverage needs in accordance with established procedures.
• Support emergency procedures and coordinate staff responses until nursing, administration, or emergency personnel assume responsibility.
• Assist with orienting, training, coaching, and evaluating new and existing staff.
• Provide clear shift-to-shift reports and communicate unresolved concerns, follow-up needs, and significant events to the incoming supervisor and nursing staff.
• Maintain professional boundaries and protect resident privacy, dignity, safety, and confidentiality at all times.
• Perform direct-care assignments when necessary to meet residents' needs and maintain safe operations.
• Immediately report suspected abuse, neglect, mistreatment, exploitation, or unsafe conditions in accordance with facility policy and applicable reporting requirements.
• The Senior HHA - Shift Supervisor may provide direction, coaching, and documentation but may not independently issue formal disciplinary action unless specifically authorized by administration. Resident-Care Duties Provide personal care within the employee's training, credentials, assigned responsibilities, and permitted scope, including:
• Assist residents with showering and personal hygiene.
• Assist with hair care and shampooing.
• Assist with denture and mouth care.
• Make unoccupied beds and change linens as assigned.
• Assist residents with dressing, including upper and lower garments, shoes, and socks.
• Apply and remove TED stockings when assigned and permitted.
• Provide toileting assistance, reminders, and incontinence care as assigned.
• Assist with mobility, transfers, positioning, and ambulation in accordance with the resident's care plan and the employee's training.
• Encourage resident independence and choice while providing necessary support. Special Care Assigned by the RN Perform or assist with the following only when assigned, trained, and permitted by the employee's credentials and applicable requirements:
• Obtain and record temperature, pulse, and respiration.
• Assist with exercises that have been demonstrated by the RN or registered therapist.
• Assist residents with wheelchairs, walkers, crutches, hearing aids, and other approved devices as demonstrated.
• Assist only with medications and document assistance on the appropriate medication observation record, as permitted.
• Assist with simple, routine eye drops when assigned and permitted.
• Observe and document a diabetic resident's self-administration of blood glucose testing and insulin, as permitted.
• Cue and remind residents regarding meals, activities, toileting, appointments, and other identified needs.
• Complete other assigned tasks consistent with the resident's plan of care and the employee's verified qualifications. Environmental and Housekeeping Duties Perform light housekeeping duties necessary to maintain a clean, healthful, and safe environment, including:
• Complete resident laundry and assigned bed-linen changes.
• Clean resident refrigerators and complete food inspections as assigned.
• Pick up and properly dispose of trash and waste.
• Clean resident equipment as assigned.
• Identify and report environmental, maintenance, infection-control, and safety concerns.
• Help ensure resident apartments and common areas remain orderly, safe, and respectful. Resident Support and Documentation
• Promote residents' mental alertness and social engagement through conversation, reading, games, and other appropriate activities.
• Provide simple emotional and psychological support as needed.
• Observe residents and immediately report changes in their mental, emotional, behavioral, or physical condition to the responsible nurse.
• Accurately and promptly document the completion of assigned duties in residents' personal-care logs and other required records.
• Maintain the confidentiality and security of residents' and employees' protected health information.
• Communicate professionally and respectfully with residents, families, visitors, coworkers, and members of the interdisciplinary team. Professional and Compliance Expectations
• Perform all duties according to high ethical, professional, and moral standards.
• Follow the facility's corporate compliance plan, confidentiality requirements, policies and procedures, and all applicable governing requirements.
• Maintain appropriate professional boundaries with residents and families.
• Remain attentive and available throughout the assigned shift.
• Use personal cell phones only in accordance with organizational policy.
• Take breaks only in designated staff break areas.
• Refrain from using resident apartments or resident common areas for personal activities or unauthorized breaks.
• Report to work prepared to carry out both direct-care and supervisory responsibilities.
• Perform other duties assigned by the LPN, RN, Vice President of Assisted Living Services, or other authorized administrator. General Statement The Senior HHA - Shift Supervisor is a working supervisory position responsible for supporting resident care and overseeing the HHA, PCA, and LPN staff on duty during the assigned shift. The position provides operational and administrative supervision to staff; clinical observation and direction remain under the RN. The Senior HHA - Shift Supervisor communicates directly with the Administrator on Call, who will be either the RN or the Vice President of Assisted Living Services, regarding resident concerns, staffing issues, emergencies, incidents, policy violations, and other significant events. Supervisory and Shift Leadership Duties
• Supervise, direct, and support all HHA, PCA, and LPN staff on duty during the assigned shift.
• Serve as the primary staff leader and point of contact for the shift.
• Coordinate staff assignments and ensure appropriate coverage of resident-care and operational responsibilities.
• Conduct shift huddles and communicate assignments, priorities, resident concerns, and relevant updates.
• Communicate directly with the Administrator on Call-the RN or Vice President of Assisted Living Services-regarding staffing concerns, resident changes, incidents, emergencies, policy violations, and significant operational issues.
• Contact the Administrator on Call promptly whenever an issue requires clinical guidance, administrative direction, or additional support.
• Complete required daily audits accurately and within established timeframes.
• Daily audits may include resident-care documentation, personal-care logs, medication observation records, staffing assignments, safety rounds, infection-control practices, environmental conditions, and other records or areas assigned by nursing or administration.
• Correct minor deficiencies when possible and immediately report significant findings to the Administrator on Call.
• Document audit findings, corrective actions taken, unresolved concerns, and required follow-up.
• Monitor staff attendance, punctuality, appearance, conduct, and completion of assigned duties.
• Make regular rounds throughout the building, resident apartments, and care areas to monitor resident safety, staff performance, and compliance with organizational expectations.
• Verify that resident-care assignments and required documentation are completed accurately and on time.
• Enforce organizational policies, including policies governing personal cell phones, designated break locations, confidentiality, resident rights, workplace conduct, and the appropriate use of resident common areas.
• Immediately redirect staff when policies, care plans, assignments, or performance expectations are not being followed.
• Document and report policy violations, performance concerns, resident-care issues, incidents, and unusual occurrences.
• Promptly notify the RN and Administrator on Call of changes in a resident's physical, mental, emotional, or behavioral condition.
• Assist with managing callouts and staffing coverage in accordance with established procedures.
• Coordinate staff responses during emergencies until the RN, Administrator on Call, or emergency personnel assume responsibility.
• Assist with the orientation, training, coaching, and performance monitoring of new and existing staff.
• Provide a clear shift report to the incoming supervisor, including unresolved concerns, audit findings, required follow-up, and significant events.
• Perform direct-care assignments when necessary to meet residents' needs and maintain safe operations.
• Perform other supervisory or operational duties assigned by the RN, Administrator on Call, or Vice President of Assisted Living Services. Candidates must meet at least one of the following requirements:
• Hold a current Home Health Aide (HHA) certification;
• Hold a current Licensed Practical Nurse (LPN) license; or
• Have extensive experience providing senior care and be willing to successfully complete the organization's required certification program within the established timeframe. Candidates who are not yet certified may perform only those duties permitted by their current qualifications and may not independently perform HHA or licensed nursing duties until all required credentials have been obtained and verified. The Senior HHA - Shift Supervisor must:
• Be at least 18 years of age and demonstrate maturity and emotional and mental stability.
• Be able to speak, read, and write English and communicate effectively with residents, families, visitors, and staff.
• Be able to understand and carry out directions, communicate instructions, record messages, and maintain accurate records.
• Demonstrate compassion, patience, professionalism, sound judgment, and a respectful attitude toward residents.
• Demonstrate leadership skills and the ability to direct, support, and hold staff accountable.
• Be in good physical health, as indicated by documentation maintained in the personnel file.
• Maintain all required licenses, certifications, training, and annual in-service education applicable to the position.
• Maintain current CPR and first-aid certification when required by the organization.
• Be dependable and available to respond appropriately to staffing, resident-care, and building concerns during the assigned shift.
