Position Overview:

The Activities Assistant role provides routine daily activities and services to each assigned resident in accordance with their assessment and care plan, as directed by supervisors.

• Plan and facilitate recreational, physical, educational, social, and spiritual activities based on residents’ interests, needs, and abilities.
• Encourage and assist residents with participating in group and one-on-one activities according to their level of functioning.
• Perform room visitations and activities as required by schedule and / or observed resident need.
• Transports residents to assigned locations for activity.
• Monitor and supervise residents during activities to ensure their safety and communicate any concerns with nursing and other staff.
• Maintain clear and accurate documentation of residents’ participation and responses in activity logs and resident charts, following facility policies and state regulations.

Qualifications:

• High school graduate or equivalent required.
• Must have at least 6 months of experience organizing and providing planned group activities in a long-term care setting or related environment.
