Registered Nurse Assessment Coordinator (RNAC) - Dover, DE

At Bay Terrace Rehabilitation and Healthcare Center LLC , we are dedicated to providing exceptional skilled rehab services to meet the unique needs of our patients. As a valued member of our team, we are seeking a highly skilled and dedicated Registered Nurse Assessment Coordinator (RNAC) to join our team in Dover, DE.

About the Role:

As a Registered Nurse Assessment Coordinator (RNAC), you will be responsible for providing accurate MDS assessments in accordance with state and federal regulations.

Responsibilities:

  • Ensuring the interdisciplinary team completes designated sections of the MDS assessment, and the MDS is coded accurately with information obtained via medical record review and completed in a timely manner.

  • Coordinate the RAI process, ensuring compliance with Medicare, Medicaid, OBRA, and other payor program requirements

  • Collaborate with the healthcare team to ensure seamless communication and coordination of patient care

Qualifications:

  • Current RN license valid in the state of Delaware

  • Experience as an MDS Coordinator
