Community Care Hub Navigator & Admin Specialist
Better Health Together
Better Health Together is seeking a temporary CCH Administration and Navigation Coordinator to support the region with referrals, eligibility checks, and data management. The role covers rural, tribal, and urban areas within the BHT region, including mileage reimbursement. You will partner with Health Benefit Exchange and Navigator programs to determine eligibility and enroll in multiple programs. Ideal candidates have care coordination experience, a Community Health Worker certificate, and #J-18808-Ljbffr Better Health Together
