Family Practice - Without OB Physician

AdventHealth Well 65+ is seeking an experienced, Board Certified Family or Internal Medicine to join our team and provide personalized care to patients 65-years of age and older. This practice exclusively focuses on Medicare Advantage patients. At work, you will experience: More Time with Patients: Up to 40 minutes with patients to provide comprehensive, personalized care Smaller Patient Panel. Max patient panel of 700. Lower Daily Patient Census. 14-16 patients per day. Clinic Support Team. Includes RN Case Management, 1.5 Health Coaches (MA s) per provider, Social Worker, Pharmacy, Care Coordinators, Welcome Coordinator, HCC Coding Support, and Administrator. Education and Experience Required: Completion of a US-based Internal or Family Medicine Residency Experience in a Value-Based Care practice Licensure, Certification, and Registration: Current, valid Florida medical license (or ability to obtain) Board Certification in Internal or Family Medicine by ABMS or AOA
