Occupational Therapist - Home Health

This is a 13-week contract position.

Day shift, 5x8-hour (08:00 - 17:00).

As an Occupational Therapist in a home health setting, you will be responsible for providing skilled therapy services to patients in their homes, focusing on recovery and rehabilitation. Your role includes performing evaluations, developing treatment plans, and assisting patients with activities of daily living, cardiac rehabilitation, and other therapy interventions.

Responsibilities:

• Conduct thorough evaluations and assessments of patients' needs and functional capabilities.
• Establish and implement personalized treatment plans tailored to individual patient goals.
• Assist patients in improving skills for daily living and working towards rehabilitation goals.
• Perform specialized techniques including edema massage, splinting, and joint mobilization.
• Educate patients and families on safety, injury prevention, and home exercise programs.
• Document therapy services accurately and maintain compliance with healthcare regulations.
• Coordinate care with interdisciplinary team members as needed.

Required Experience / Certifications / Licensure:

• Minimum of 1 year of experience in home health care; 2 years preferred.
• Valid state occupational therapy license and CPR certification (BLS).
• Experience with OASIS preferred.
• Must have a valid driver's license and reliable transportation for home visits.
• COVID-19 vaccination or approved exemption required.
• Flu shot or declination required, with mandatory mask-wearing while at work.
