Key Responsibilities\n

  • Conduct in-depth assessments of clients’ mental and emotional health, compiling comprehensive histories and identifying needs to create personalized treatment plans.

  • Implement therapeutic interventions drawing from solution-focused and evidence-based methodologies to foster positive health outcomes.

  • Maintain accurate, organized, and confidential client records in adherence to HIPAA regulations and company policies.
  • \n
  • Collaborate closely with a multidisciplinary treatment team, including nurses, physicians, and other social workers, to coordinate care and monitor client progress.

  • Stay abreast of the latest research and best practices in social work and hospice care, applying new insights to enhance service delivery.

Patient Population / Client Focus\n

This position focuses on serving individuals receiving hospice care, along with their families. You will work with patients experiencing a range of psychological adjustments associated with terminal illnesses, where your empathetic approach will be crucial in helping them navigate grief, fear, and acceptance. By offering support, education, and counseling, you directly contribute to improving their quality of life and facilitating smoother transitions.

Qualifications\n

  • A Master’s degree in Social Work from an accredited institution is required.

  • Active LCSW licensure in the state of California.

  • Strong communication and interpersonal skills, coupled with effective problem-solving abilities.
