Nurse Case Manager: Hospital Discharge & Care Coordination

Ochsner Health in New Orleans, LA seeks an experienced nurse to lead discharge planning and utilization review within the hospital care team. You will review admissions, coordinate continued stay decisions, and liaise with ED physicians, hospitalists and payers to ensure medically appropriate care. The role requires an active Louisiana RN license and 3 years of hospital experience; familiarity with CMS guidelines and payer rules is a plus. #J-18808-Ljbffr
