Job Title: Direct Support Professional                                                                                       Salary: 10.00/ hourly

Job Summary

• Cares for elderly, disable, convalescent, or handicapped at individuals homes or day-time residence

General Responsibilities

• Read, understand, and Implement individual support as written.
• Provides assistance and support to individual(s) in activities of daily living
• Teach appropriate skills and involve individual(s) in activities supported in meaningful and valued activities
• Provides and seeks out opportunities for individual(s) supported to make good choices
• Obtain knowledge of individual(s) supported desired outcomes and works to ensure they are integrated into daily activities
• Incorporates time, space, and opportunity for privacy in the lives of individuals supported
• Assist with purchasing groceries and preparing meals for the individual
• Administer oral medication written in ISP and MAR (Must Be Level 1 Medication Administration Certified)
• Visit several households to provide similar daily care
• Complete daily documentation logs, charts, and billing sheets

Equipment Operated

• Will be handling standard housing appliance and electronics. As well as wheel chairs, walkers, etc.

Qualifications

• Must be 18 years old or older
• High School Diploma or equivalent
• Ability and willingness to support individual(s) with disabilities and/or challenging behaviors

Confidential Data

• Must maintain strict confidentiality of all personal information concerning individual(s) supported.
• Must know, advocate, and respect the rights of individual(s) supported
• Maintains confidentiality of records/information according to HIPAA, State, Federal Laws and Guidelines.

Usual Physical Demands

• Must occasionally lift and/or move the individual(s) supported and/or up to 15 pounds
