Active Ohio Physical Therapist license required prior to start. Minimum one year of home health physical therapy experience required. Experience with HCHB (HomeCare HomeBase) documentation required. BLS certification from AHA or ARC may be required. Ability to perform Start of Care visits, evaluations, recertifications, and treatment visits in home settings. Reliable transportation for patient visits. Mileage reimbursement provided between visits. Preferred: Lymphedema therapy experience. Assignment Details Facility Type: Home Health. Shift Name: Days. Shift Details: Days . 5x8 schedule. Weekly Hours: 40 hour guarantee. Coverage Areas: Primary coverage includes Pickaway, Fairfield, Ross, Muskingum, Licking, Perry and Hocking counties as assigned. Documentation Platform: HCHB (HomeCare HomeBase). On Call/Weekend Requirement: None. Dress Code: Scrubs; no specific color required.
