Job Summary:
Physical Therapist (PT) specializing in Home Health (HH) services in a home-based setting. This role involves providing therapy to a diverse patient population across various age groups, managing multiple comorbidities, and ensuring timely documentation of patient visits. The position offers a Monday through Friday schedule with daytime hours.

Job Details:

• Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with a minimum of 40 hours per week
• Required Qualifications: Valid PT license, BLS (Basic Life Support) certification; at least 2 years of relevant experience; valid drivers license and car insurance; willingness to complete all required modules prior to arrival, including MyJourney modules
• Preferred Qualifications: Prior home health experience is highly recommended unless otherwise specified
• Responsibilities: Conduct physical therapy sessions for all ages, manage patients with multiple comorbidities, complete documentation within one day of each patient visit using HomeCare HomeBase electronic medical record system
• Additional Information: Regular PT work with flexibility for self-scheduling; manage patient care responsibilities in a home health environment, with all required certifications maintained throughout the assignment; manager or supervisor available 24/7 for support; shifts of 8 hours, 5 days per week, from 10/12/2026 to 01/11/2027
